The escalation protocol
You write it; we execute it without improvising. What it typically contains:
- The rotation, by day and by shift, and how far in advance it’s set
- Contact order for each person: mobile, backup, then the fallback
- The interval between attempts, and how many rounds before moving on
- The situations that trigger escalation, in the patient’s likely words
- The situations that never do, however insistent the caller
- The instruction to direct callers to 911 — the exact wording, and exactly when
- Who gets told if the protocol runs out of contacts
Rotation changes are made in the portal and apply to the next call. Nothing depends on someone remembering to tell us.
The log is the deliverable
Every escalation attempt is timestamped: which number, at what time, connected or not, and what happened next. If a patient later says nobody called them back, there is a record rather than a recollection.
The same log tells you something more useful over time: how often the rotation is actually disturbed, and how many of those calls met your own criteria. Most practices find the second number smaller than they feared and adjust the threshold accordingly.
Where our judgment stops
We don’t triage. We don’t assess symptoms, discuss medication or results, or decide on clinical grounds whether something can wait. The receptionist matches the call against your written list and acts on it — and where the list says a symptom means 911, that instruction is given immediately and without hedging.

What happens if the on-call physician doesn't answer?
We work down the contact order at the interval you set, for as many rounds as your protocol specifies, then notify whoever your protocol names as the fallback. Every attempt is timestamped in the log.
Can we change the on-call rotation ourselves?
